If you need it, we can provide a more in depth conversation about “What matters to you” and can carry out an assessment of your support needs. This will help to identify solutions to meet your assessed needs.
Following this, depending on your identified solutions, we can either give you information and advice to help you make your own arrangements or may refer you for further support through Social Services.
Depending in your needs, we might take a multi-disciplinary approach to your assessment and support, for example involving health colleagues.
Through Social Services, we may be able to support you to look at housing and accommodation options, including more major adaptations if required.
Other support could include:

- Help to communicate, or build and maintain relationships with others.
- Support to access the community, education, employment or leisure activities.
We will always seek to promote your independence and reduce your reliance on formal support services as much as possible.
